Homecare Anaphylaxis Protocol

Adults:

  • Stop IV & Call 911
  • Give Epinephrine (1:1000) 0.5ml SQ or
  • Total amount of epinephrine automatic Injector SQ
  • May repeat in 10-15 minutes if needed
  • Change IV tubing & Start NS
  • Administer Diphenhydramine (Benadryl) IV  50mg or IM if no IV access
  • Place patient supine with legs elevated
  • Initiate basic Life support & Monitor VS
  • Wait for EMS/Notify MD ASAP

Anaphylaxis Kit contents:Epinephrine ampule 1:1000 or Epinephrine automatic injector

diphenhydramine hydrochloride (Benadryl) injectable ampule

Normal Saline for IV infusion (minimum 250cc)

IV administration set; IV start kit

TB syringes with needles; filtered needles; Alcohol wipes.

Pediatric

1.      Epinephrine 1:1000 0.01ml//kg or as follows:

Less than 1 year0.05ml SQ
1-4 years0.15ml SQ
5-9 years0.2ml SQ
10 years & older0.3 – 0.5ml SQ

Or Total amount of epinephrine automatic injector SQ
May repeat in 10-15 mins if needed.

2.      Give Diphenhydramine (benadryl) IV or IM if no IV access:

1.5mg/kg or:

Less than 2 years12.5mg
2-4 years25mg
5-11 years37.5mg
12 years and older*50mg

*follow Adult procedure; Administer NS infusion with new IV tubing if IV access available or start peripheral line.
